Perry Lutheran Homes is looking for caring QA/IP/Wounds Nurse (RN) to take great care of our loved residents!
Position and Shift Offerings:
Full-time:
- Monday-Friday
- 8am-4pm (flexible)
- Occasional On-Call (few times a month)

- Assure that effective quality nursing care is delivered which is outcome focused through utilization of the nursing process
- Develop and communicate individualized plan of resident-centered care in collaboration with the resident/responsible party, family, and care team
- Implement plan of care consistently, effectively and cost efficiently with focus on resident centered outcomes by utilizing all appropriate and available resources for resident/family/responsible party counseling and discharge planning
- Work in collaboration with physician and/or other health care professionals by sharing information relevant to changing plan of care
- Supervise and lead CNAs to perform safe, compassionate, and high-quality care
- Perform accurate and efficient charting

Coordinate the facility's Quality Assurance and Performance Improvement program in alignment with CMS guidelines.

Collect, analyze, and report quality data (falls, infections, pressure injuries, rehospitalizations, etc.).

Identify trends, develop action plans, and lead root cause analyses (RCAs) in collaboration with leadership and care teams.

Facilitate monthly QA meetings and maintain documentation for survey readiness.
-
Serve as the designated Infection Preventionist (IP) per CMS regulations.
-
Develop, implement, and monitor infection prevention and control policies based on CDC, APIC, and state guidelines.
-
Lead infection surveillance, outbreak investigations, and antibiotic stewardship initiatives.

Educate staff on standard precautions, PPE, transmission-based precautions, and hand hygiene.

Report communicable diseases to public health authorities as required.

Assess, document, and oversee treatment of pressure injuries, surgical wounds, skin tears, and other wounds.

Collaborate with interdisciplinary team and physicians to develop wound care plans.

Provide direct care as needed and monitor wound healing progress.

Educate staff on skin integrity practices, turning/repositioning, and prevention of pressure injuries.

Monitor use of support surfaces, heel protectors, and offloading techniques.
Requirements:

Minimum of 2 years nursing experience in long-term care or geriatric setting

Experience in QAPI, infection prevention, or wound care

Knowledge of state and federal long-term care regulations

Strong critical thinking, data analysis, and teaching skills
- Applicants must be 18 years of age or older
- Desire to work with and serve residents in a way that is compassionate, personalized, and respectful
- Applicants must be a Registered Nurse (RN) in good standing with the State of Iowa and maintain a current certification throughout employment

Nurse (LPN/RN)

Full-time:
- 6PM-6AM (Weekend Package Available for RN)
- 6PM-6AM (Mondays, Tuesdays, and Thursdays)
- 2PM-10PM
Part-time:
- 6AM-6PM
- 6PM-6AM
PRN available for all shifts!
Shift Differential Pay: 10% more for 2pm-10pm, 5% more for 10pm-6am
