2022 Excellence in Leadership Award

Presented by LeadingAge Iowa to Perry Lutheran Homes' Guest Experience Director, Tonya Swank, for her innovating, creative and dedicated leadership as she finds ways to create outstanding and memorable moments for residents and inspires those she leads to do the same.

2021 Caregiver of the Year Award

Presented by LeadingAge Iowa to Perry Lutheran Homes' Team Member, Inge Smothers, Certified Med Aide (CMA) for her compassion and joy in caring for and meeting the needs of our residents. Not only is Inge an excellent CMA with great skill and exceptional care, she is a beloved friend to residents and their families. Inge pays attention to the details that matter and goes above and beyond basic care to bring love and joy into residents' lives.

2020 Best Memory Care Community

Presented to Perry Lutheran Homes' by MemoryCare.com for unique and innovative CCDI memory care neighborhoods, St. James and St. John's, as well as the approach to care and specialized dementia training for staff located within the Des Moines area.

2018 LeadingEdge Care & Services Award

Presented by LeadingAge Iowa to Perry Lutheran Homes' for the Meaningful Mission Projects Program that provides meaning, purpose, and value to the lives of residents, volunteers, and staff. This gives residents the opportunity to stay involved in community as well as doing good for and giving back to others.

2015 Phoenix Award

Presented by LeadingAge Iowa to Perry Lutheran Homes' CEO, Rev. Max Phillips, for innovative ideas, creative solutions, and support of staff initiatives and development. Rev. Phillips also serves as the chaplain for Perry Lutheran Homes and integrates worship, bible study, prayers and faith into the daily lives of all who live and work at PLH.

2022 Business of the Year Award

Presented to Perry Lutheran Homes by the Perry Chamber of Commerce for serving as an example to others through leadership, service and commitment. PLH continues to expand housing and services to older adults amidst the COVID-19 pandemic. And, leads the way in care and concern for all elderly residents in Perry - including those living in their own homes - through "Project Deliver the LOVE" and Meals on Wheels.

2021 Excellence in the Workplace Award

Presented by LeadingAge Iowa to Perry Lutheran Homes' for the launch and execution of the StepUp Career Advancement Program to build a stable, direct care workforce of highly trained, qualified, and committed nurses, nurse aides, and other long-term care staff at Perry Lutheran Homes.

2019 Public Trust Award

Presented by LeadingAge Iowa to Perry Lutheran Homes' for the StepUp Movement that inspires others in the community to StepUp (Serve Together our Elderly with Passion) and take notice of older adults in need and to take action so that no elder is ever lonely, afraid, hungry or forgotten.

2016 Caregiver of the Year Award

Presented by LeadingAge Iowa to Perry Lutheran Homes' Team Member, Tonya (Wadle) Swank, CMA and Restorative Aide for her dedication to serving residents, her positive attitude, and revamping the restorative program. Tonya holds a special place in her heart for residents with dementia finding ways to help them live meaningful lives.

2015 Best Innovation in Aging Services & Housing Award

Presented by LeadingAge Iowa to Perry Lutheran Homes for the St. John’s Memory Care neighborhood with its unique and innovative Sensory-Connected Environment promoting a more home-like and peaceful environment for a sense of calm and contentment.

2021 Public Trust Award

Presented by LeadingAge Iowa to Perry Lutheran Homes' for "Project Deliver the LOVE" - an event that celebrates our elderly citizens and also raises awareness for the silent epidemic of elder loneliness, helps community members make and form lasting and meaningful connections, and engages churches, businesses, service organizations, and community members in outreach to our elderly citizens impacting over 400 older adults each year.

2021 Excellence in Leadership Award

Presented by LeadingAge Iowa to Perry Lutheran Homes' COO and Administrator, Melissa Gannon, for her passion and dedication to lead successfully and simultaneously through the COVID-19 pandemic, the damaging derecho, and the acquisition of a struggling senior living community.

2019 Board Member of the Year Award

Presented by LeadingAge Iowa to Perry Lutheran Homes' Board Member, Mr. Bill Van Vliete for giving of his time and talents to help make Perry Lutheran Homes' King's Gardens Campus feel like a home through design, color, furniture, flooring and more to support and improve resident quality of life.

2016 Leading Edge Care and Services Award

Presented by LeadingAge Iowa to Perry Lutheran Homes for the "Remembering Hope" Alzheimer's Education & Outreach Program. This program was designed to equip and educate church and community about Alzheimer's and dementia, and to bring support and hope to those caring for someone with dementia.

2014 Best Innovation in Aging Services & Housing Award

Presented by LeadingAge Iowa to Perry Lutheran Homes for the St. James Memory Care neighborhood with unique and innovative Life-Skills Environmental Stimulation increasing the quality of life and care of residents with dementia.
