  • Functional literacy in written and spoken English.

  • Medication administration or Certified Medication Aide experience with proof of Department of Health Services approved medication administration program listing on state registry.
  • Experience with electronic records and ability to write reports and correspondence (Point Click Care experience preferred).
  • Physical and emotional capacity, education and experience to respond to community member needs.
  • Physical demands such as; standing, walking, sitting, bending, climbing stairs, hearing, seeing, kneeling, lifting up to 35 pounds or more.
  • Ability to effectively communicate with confidence, clarity, precision, and professionalism.

Activity Director

Perry Lutheran Homes is looking for friendly and energetic, upbeat, people like you to coordinate activities for our residents in our Christian caring communities and out in public as an Activity Director for our King's Gardens Campus.

Position and Shift Offerings:

  • Monday-Friday

How You'll Make an Impact:

  • Assesses residents in relation to their interests, talents, previous lifestyles, and time use patterns. Organizes and develops this information in order to contribute to the comprehensive assessment and care planning process as a part of the facility's interdisciplinary care planning team.
  • Provides opportunities for entertainment, exercise, relaxation, and expression for all residents. Assists the physically, emotionally and cognitively impaired individual and family to explore appropriate leisure interests.
  • Develops activity care plans which assure that all residents are provided with recreational activities that are consistent with the needs and wishes of the resident groups, individual residents, bed-bound residents, self-sufficient residents, and the community as a whole. The activity director assures that this care plan is integrated into the overall plan of care for each individual resident.
  • Establishes community and facility resources that can contribute to the activity program.
  • Creates and ongoing calendar of activities, which creatively meets multiple needs and is accessible and appealing to both men and women of all ages and abilities. Provides a copy of the calendar to each resident’s room, staff offices, and at least one large calendar displayed in facility.
  • Maintains records relating to the participation of each resident in activity programs and documents resident acceptance or lack of acceptance to these activities.
  • Secures materials, equipment, transportation (as required), entertainment, refreshments, program leaders, volunteers, and anything else that might prove necessary in order to adequately provide for scheduled activities.
  • Communicates and coordinates with other department heads and staff in order to assure the success of planned activity programs.
  • Evaluates activities and programs in order to determine resident outcomes. Adjusts activities as necessary to increase the probability of successful outcomes.
  • Develops and operates within a budget for the activities program.
  • Recruits, trains, directs, utilizes, and provides periodic recognition for individuals who volunteer in the activity program.
  • Utilizes staff, family, volunteers and community resources in the completion of the job duties and responsibilities.
  • Provides at least one in-service education program per calendar year regarding the activity program, working with the cognitively impaired, and/or other related topics.

Requirements:

Home Care Director (RN)

Home Care Director (RN):

Full Time:

How You'll Make an Impact:

  • Demonstrates knowledge of client's health; oversees and ensures the design and implementation of quality individual and collective client's care treatment, programs, plans and services.
  • Closely monitors accrediting/surveying/regulatory body criteria to maintain compliance.
  • Would assist LTC nursing until caseload is built for full-time work.
  • Assists in the assessment of and planning of client care plans; ensures adherence to physician orders and other professional health-care treatment plans.
  • Reviews physicians’ orders for prompt implementation. Evaluates all client’s medication sheets, treatment sheets and other charting for accuracy. Ensures home care services complete appropriate charting and documentation of resident care.
  • Proactively identifies opportunities for improvement and enhancement of policies; implements all approved policies.
  • Ensures appropriate inventory of supplies and equipment within budget guidelines.
  • Develops, and implements quality management, utilization reviews and discharge planning programs.
  • Leads by example, fosters open communication and resolves conflict in a constructive manner. Manages and coaches’ home care staff; establishes and maintains culture that supports the mission/vision of the organization and facility.

RN Requirements:

  • RN graduate from accredited school of nursing (required).
  • Self-starter as they will be developing/marketing this program from the ground up (required).
  • Home care/home health care experience (required).
  • One year full-time supervisory/management nursing experience (required).
  • CPR Certification (required).
  • State of Iowa RN licensure by Iowa Board of Nursing (required).
  • BSN (preferred).
  • Must have strong business and financial management skills, knowledge of reimbursement requirements and regulations, strong critical thinking and analytical skills, and strong verbal and written communication skills.
  • Ability to set and achieve relevant and challenging goals for self and team.
  • Ability to hire and develop a quality home care team.
